Description

This service is aimed at reducing Urgent and Emergency Care service use in identified individuals through holistic case management and support. This is not a crisis response service, but will work on coordinating the actions of providers across the entire health and social care system to ensure that patient’s needs are met.Barca Outreach Support Team (also known as High Intensity Service User Service)
